This notice explains how information submitted through the booking request form is collected and used.

Who collects your information

Dr Integrity Mchechesi and Dr Lerm's Surgery use the form to receive appointment requests. You can contact the practice at 02 6654 0633 or docs@drlerm.com.au.

What we collect and why

The form collects your name, phone number, and preferred appointment date and time. These details are collected so reception can contact you to confirm an appointment or offer an alternative. Please do not submit symptoms or other clinical information through this form.

What happens if you do not provide it

The practice will not be able to respond to an online request without your name, phone number and preferred time. You can instead call the practice to request an appointment.

Use, disclosure and storage

Your information is used for appointment administration and is available to authorised practice staff and service providers that support the website and its secure hosting. It is not collected for direct marketing. Hosting providers may process or store information outside Australia.
